Choosing Between Central and Local Labs in Clinical Development
Strategy first: what “central vs. local” really solves—and how to decide for your study

A central lab strategy concentrates testing in one or a small number of global facilities using harmonized methods, reagents, and reference ranges. Decide with criteria that map directly to risk and value. Start with assay criticality: If an analyte feeds a primary endpoint or stratification, keep it central to control analytic variance. For exploratory biomarkers with evolving methods, centralization also simplifies revalidation waves. For urgent safety tests (potassium, troponin) where delays endanger subjects, local capacity with strong guardrails is often superior. Next, consider geography and logistics risk: long lanes, remote sites, and complex customs regimes (e.g., human material biospecimen export permits) raise temperature-excursion probability; for such lanes, a local option mitigates temperature excursions monitoring burden. Whatever you choose, write the rationale in your Lab Strategy Memo and connect it to your risk register: the trade-off (e.g., speed vs comparability) should be explicit, owned, and testable with key risk indicators (KRIs).
Two decisions round out strategy. First, decide whether PK/PD or advanced biomarker work belongs in the central or a specialty partner. Even in a centralized model, ultra-specialized assays (flow cytometry, genomics) may ride dedicated lanes with their own kits, chain-of-custody, and stability claims—govern them as “central-like” even if done by a niche vendor. Second, decide where “clinical significance” logic lives. If local safety labs remain in play, define a uniform medically significant findings workflow and thresholds that bind all sites, and require local labs to report critical values within a set TAT to the PI and medical monitor with auditable confirmation. These two choices often determine whether your hybrid actually behaves like a coherent system.
Make the lane work: kits, barcodes, packaging, and chain of custody that never cracks
Once strategy is set, reliability lives in the lane. A robust kit program is the backbone of execution. Start with unified kit design and barcoding that encodes protocol, subject, visit, time-point, and analyte in a scannable label set. Pair primary tubes and aliquots with pre-printed identifiers and tamper-evident seals to simplify chain of custody documentation and reduce transcription risk. Include pictorial IFUs and a one-page “what to do when…” job aid for common failure modes (mislabeled tubes, delayed couriers, hemolysis). For PK/PD and biomarker tubes, specify anticoagulant, fill volumes, inversion counts, processing windows (centrifuge g-force/time), and freeze requirements; these details drive sample stability and packaging and must be non-negotiable at sites.
Design packaging around physics, not hope. Every shipper should have qualified insulation, phase-change materials appropriate to ambient weather, and validated hold times for 2-8°C, −20°C, or ≤−70°C lanes. Mandate time-temperature indicators or loggers for all frozen shipments and at least initial-phase ambient pilots; require the central to publish a stability budget for each analyte so sites know maximum room-temperature exposure pre-freeze. “Excursion permitted/not permitted” rules belong on a single laminated card in every kit. When an excursion occurs, the site or courier must annotate, photograph, and escalate per the temperature excursions monitoring SOP; the lab then decides usability with a documented rationale referencing method stability data.
Move packages with intent. Courier SLAs must specify pick-up windows, transit time, weekend/holiday coverage, and contingency options. Where export/import applies, pre-clear biospecimen export permits, tariff codes, and consignee documentation to prevent customs holds; share a simple one-pager with sites so paperwork errors do not become biological ones.
Reduce rework before it starts. Publish specimen acceptance/rejection criteria (hemolysis, clotting, underfill, thawing, label mismatch) and provide sites with a photo gallery of “reject vs. accept with comment.” Run early “process validation” days during first-patient-in to catch quirk combinations of tubes, centrifuges, and staff: this is cheaper than discovering month-two that your plasma is routinely hyperlipemic. Give sites a short feedback loop—weekly error dashboards by site with practical fixes, not just admonitions. Nothing improves performance like seeing last week’s root cause and this week’s drop in rejects.
Finally, lock down critical value reporting TAT. Whether central or local, establish analyte-specific bands (e.g., potassium ≤2 h from receipt; troponin ≤1 h from analysis) and the escalation ladder (call PI → call sponsor medical monitor → document in EDC note to file). Store call logs, timestamps, and recipients in the lab portal and file summaries in the eTMF. This is where hybrid models often fail: a fast local result is worthless if the call tree is unclear; a precise central result is dangerous if it arrives after a dose decision. Treat TAT as a safety control with owners, not a hope with averages.
Make the data sing: harmonization, DTAs, reference ranges, and reconciliation with zero drama
Lab lanes succeed when data arrives standardized, traceable, and ready for analysis. Write a crisp data transfer agreement DTA with every provider—central or local—that fixes file formats, encryption, cadence, error reporting, and resubmission rules. Define transport (SFTP with checksum), change control for layout updates, and sunset rules for superseded files. Assert the mapping layer up front: every analyte gets a LOINC coding and harmonization assignment, unit standardization (SI is safest), and method metadata. Preserve original units and results in a raw layer, then expose standardized values to EDC or the lab hub that feeds your CDISC LB domain mapping. Reference ranges can sabotage otherwise orderly pipelines. Create a single registry that stores per-lab, per-method, per-sex/age reference intervals with version dates, then generate the range embedded in each result record at load time. For hybrid models, run a reference range standardization exercise for the top 30 analytes: compare local ranges to the central’s, confirm clinical equivalence or flag deltas, and decide whether to display “study ranges” for interpretive consistency. Document the rationale either way—this is a frequent query during inspections and medical review.
Reconciliation should be boring. Automate subject/visit/time-point checks against EDC and IWRS; raise discrepancies only when the logic cannot resolve. Track missing, duplicate, out-of-window, and outlier patterns and surface them on a weekly “lab data reconciliation” dashboard by site and analyte. For medically significant findings workflow, define uniform flags and routes: a medically significant but within-range result (e.g., rapid drop from baseline) should still alert clinicians per the medical monitoring plan. For unblinded analytes, ensure role-based access and redact displays appropriately in blinded systems; nothing ends a study like accidental unblinding from an enthusiastic listing.
Quality lives in metadata. For each file, capture sending system version, instrument model, reagent lot (when available), sample integrity flags (hemolysis, icterus, lipemia), and any chain of custody documentation anomalies. Persist audit fields (who loaded, when, checksum) and route all layout changes through a documented change-control with impact analysis on downstream derivations. Store data-lineage diagrams and calculation definitions in your metrics catalog so “why does this value look different?” can be answered with a link, not a meeting.
Close with people and process. Train CRAs and data managers on lab-specific queries (e.g., unit conversions, method switches), and publish a short “lab query writing guide” so sites receive clear, respectful, and actionable requests. Link your lab dashboards into the main governance report so executives see KRIs and fixes in the same language as enrollment and monitoring. When hybrid models are in play, emphasize that consistency is a team sport: centrals, locals, couriers, sites, data management, and medical monitoring all interact. Your data will only be as standard as the least standard actor in the chain; your operating system—DTAs, mappings, ranges, reconciliation—must bring them into tune.
Governance and oversight: audits, accreditation, vendor performance, and an inspection-ready story
Govern the lab estate like a system, not a collection of vendors. Begin with accreditation. Verify and file current CLIA CAP ISO accreditation certificates for all labs and confirm scope covers the assays used. Where jurisdictional requirements differ, document equivalence or compensating controls under GCLP compliance. Schedule lab audits proportionate to risk: pre-qualification for all, on-site/remote for high-impact assays or weak history, and focused data-integrity reviews for any provider with system changes mid-study. Audit checklists should include training records, instrument maintenance, calibration, result authorization controls, Part 11 alignment for portals, and evidence of timely critical value reporting TAT.
Run vendor oversight for labs as a living process. Stand up a monthly quality forum that reviews TAT, rejection rates, temperature-excursion frequency, data-feed errors, and query aging. Use a scorecard (green/amber/red) with owners and due dates, and open CAPA for repeated misses. For hybrid estates, compare local-vs-central variance on key analytes and investigate outliers with method cross-walks or proficiency testing. Tie oversight to contracts: change-order governance for new panels, surge capacity for screening spikes, and service credits for chronic misses. A hybrid that never reconverges on quality is just two mediocre systems; your oversight must force convergence.
Make risk transparent. Record lab risks in your study risk register with KRIs and thresholds (e.g., frozen-lane excursion rate >3% per month; hemolysis rejects >2% at a site; DTA resubmission rate >1% per feed). Connect each risk to a countermeasure: supply extra coolants, switch to a faster courier lane, retrain phlebotomy, or centralize a problematic analyte. Keep inspection-readiness evidence current: Lab Strategy Memo, accreditation proofs, audit reports and responses, DTAs and mapping tables, stability/shipper qualifications, weekly metrics, and CAPA logs. Address the global context. Align safety and data principles with recognized authorities to keep your narrative credible across regions. Anchor U.S. expectations with the U.S. Food & Drug Administration (FDA); reference EU and UK laboratory guidance via the EMA; link global GCP and laboratory practice via the ICH and public health realities via the WHO. For Japan and Australia, ensure jurisdictional nuances are addressed through the PMDA and TGA. Keep one authoritative link per body in your SOPs and governance packs to avoid citation sprawl and maintain focus on primary sources.
